Our Living Well with Chronic Conditions program is designed to support you in adjusting to a diagnosis and learning practical skills to improve daily life with a chronic condition.
While we do not provide medical advice, these in-person sessions offer a supportive environment to help you work alongside your healthcare provider in managing your health more confidently.

A FREE six-week evidence-based group education program that teaches people practical skills they need to live well with a variety of health conditions.
Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with emotions that arise from having a chronic condition.
